Overall Purpose of Job:
The Diabetes Liaison Nurse, in close collaboration with the Diabetes ANP and under the guidance of the ADON, is vital in upholding evidence-based care standards for diabetes patients. This multifaceted role involves assessment, planning, implementation, and evaluation of care, coupled with leadership, education, research, and counselling responsibilities. You will be central to enhancing patient flow, service performance, and ensuring seamless transitions for patients within the diabetes service.
  
Key Responsibilities:

  • Assess, plan, deliver, and evaluate diabetes patient care, adhering to the highest clinical and quality standards.
  • Engage with the multidisciplinary team (MDT) to address patient and family needs, contributing to treatment plans.
  • Facilitate seamless service continuity by liaising with healthcare professionals, ensuring timely and safe patient transitions.
  • Provide clinical leadership and make informed decisions to elevate care standards.
  • Execute advanced nursing skills within your professional scope, leveraging specialist knowledge.
  • Mentor students and new staff, offering essential support and guidance.
  • Lead and participate in research, clinical audits, and quality improvement projects to refine patient care practices.
  • Educate and support healthcare professionals, patients, and their families.
  • Embrace additional tasks as assigned by the Assistant Director of Nursing.

  • Maintain mandatory education and a professional portfolio.
  • Uphold the nursing profession and the hospitals reputation through exemplary conduct.
  • Identify research opportunities and educational needs for patients and families.
  • Engage in performance evaluations with your line manager.

  
Educational Responsibilities:

  • Serve as a clinical resource for healthcare professionals, educating MDT members.
  • Support clinical teaching and learning initiatives within the diabetes specialty.
  • Provide health education and lifestyle management guidance to patients and relatives.

  
Other Duties and Responsibilities:

  • Model exceptional nursing care and set quality standards for the team.
  • Promote safety and hygiene, adhering strictly to hospital policies.
  • Utilize critical thinking in clinical decision-making.
  • Drive service development via protocol establishment, auditing, and data collection.

  
Person Specification:
Qualifications:

  • Current NMBI General Nurse Registration.
  • Recognized post-registration qualification in diabetes nursing.

  
Experience:

  • A minimum of five years post-registration experience in an acute care setting.
  • At least two years of experience in a role relevant to diabetes care.
  • Proven abilities in teaching, leadership, critical thinking, and multitasking.
  • Strong time management and communication skills.
