Our client is seeking an experienced Assistant Director of Nursing to join their Patient Flow and Care Transition team. This role requires someone to guarantee the safe and efficient management of patient referrals, oversee admissions, coordinate discharge planning, and serve as case manager for patients when needed.

Your Responsibilities:
- Receive and assess patient referrals from acute hospitals, collaborating with Discharge Coordinators
- Maintain accurate, concise, and informative assessments and records within the hospital, ensuring compliance with legislation standards
- Ensure proper admission procedures are followed, and all necessary documentation is completed.
- Provide ward coverage as necessary.
- Collaborate with doctors on admissions and prescriptions, ensuring that admission paperwork is completed daily.
- Follow up on patients’ outpatient appointments as required.
- Stay informed about HIQA standards.
We’re Seeking:
- Valid NMBI Registration.
- Hold a recognised Managerial Qualification (level 6 NFQ).
- Valid Staff Nurse Theatre Qualification or relevant experience in specialized areas.
- Working experience of HIQA Standards
- Experience working in elderly persons care or nursing home care
- Minimum 3 years of post-registration experience.
- Strong communication skills and a team-oriented approach.
